Despite increasing awareness, postpartum depression (PPD) remains underdiagnosed in low-resource settings. This implementation study assessed the feasibility of integrating routine PPD screening into primary care visits across 12 clinics in Peru. Over six months, 3,200 mothers were screened using the Edinburgh Postnatal Depression Scale. Screening identified 14% with probable PPD, yet only 40% received follow-up care due to limited referral infrastructure. The study demonstrates that simple, low-cost screening tools can enhance detection rates but must be paired with structured referral systems to achieve meaningful mental health outcomes.
